Healthcare Professional: Patient Referral Request for HME Awareness Program

The Atos Medical Heat and Moisture Exchanger (HME) Awareness Program is designed to help patients establish and maintain a day/night routine using HMEs, in adherence with their provider prescription, which optimizes clinical outcomes.

Patient Benefits to Enrolling the HME Awareness Program:

  • Education on the impact of 2 HMEs per day on clinical outcomes ex: coughing, sleeping, etc
  • Personalized Support to establish and /or maintain a new routine of using two HMEs per day
  • Development of Personal Goals
  • Assistance with Symptom Tracking so they can see changes from their baseline
  • Regular Contact with a Clinical Navigator to discuss challenges they may be experiencing
  • Potential for Improved Pulmonary Health after consistent use of 2 HMEs/day

Program details:

  • Components: Initial and Wrap-up Questionnaires, Weekly Contact with a Clinical Navigator, Tracking Target Measures for 4-6 weeks
  • Communication Methods: Phone, email, and/or US mail
  • HME Awareness Program can be modified to meet individual needs

Eligibility:

  • Prescription form must include a minimum quantity of 60 Provox Life HMEs/month
  • NOTE: If your patient does not meet eligibility, but remains interested, please complete the form below so we may explore personalized options with them.
