The Atos Medical Heat and Moisture Exchanger (HME) Awareness Program is designed to help patients establish and maintain a day/night routine using HMEs, in adherence with their provider prescription, which optimizes clinical outcomes.

      Benefits to Enrolling in the HME Awareness Program:

      • Education on impact of 2 HMEs per day on clinical outcomes ex: coughing, sleeping, etc.
      • Personalized Support to establish and /or maintain a routine of using two HMEs per day
      • Development of Personal Goals
      • Assistance with Symptom Tracking so you can see changes from your baseline
      • Regular Contact with a Clinical Navigator to discuss challenges you may be experiencing
      • Potential for Improved Pulmonary Health after consistent use of 2 HMEs/day

      Program details:

      • Components: Initial and Wrap-up Questionnaires, Weekly Contact with a Clinical Navigator, Tracking Target Measures for 4-6 weeks
      • Communication Methods: Phone, email, and/or US mail
      • HME Awareness Program can be modified to meet your needs

      Eligibility:

      • Prescription form must include a minimum quantity of 60 Provox Life HMEs/month
