The Atos Medical Heat and Moisture Exchanger (HME) Awareness Program is designed to help patients establish and maintain a day/night routine using HMEs, in adherence with their provider prescription, which optimizes clinical outcomes.

Benefits to Enrolling in the HME Awareness Program:
- Education on impact of 2 HMEs per day on clinical outcomes ex: coughing, sleeping, etc.
- Personalized Support to establish and /or maintain a routine of using two HMEs per day
- Development of Personal Goals
- Assistance with Symptom Tracking so you can see changes from your baseline
- Regular Contact with a Clinical Navigator to discuss challenges you may be experiencing
- Potential for Improved Pulmonary Health after consistent use of 2 HMEs/day
Program details:
- Components: Initial and Wrap-up Questionnaires, Weekly Contact with a Clinical Navigator, Tracking Target Measures for 4-6 weeks
- Communication Methods: Phone, email, and/or US mail
- HME Awareness Program can be modified to meet your needs
Eligibility:
- Prescription form must include a minimum quantity of 60 Provox Life HMEs/month
